CASE COORDINATION OF MEDICAL SERVICES

The medical case coordinator, acts as a liason between the medical community, educators, families, and community agencies in an effort to fully coordinate the services each is providing to students.

The case coordinator may assist families and teams with a variety of services. Obtaining and reviewing records of students with an extensive medical background may help teams better assess the educational needs. Identifying and working with physicians, agencies, and other community resources may assist with the coordination of information and services. The case coordinator may also provide continued support for the student by facilitating medical appointments and facilitating transportation needs of families.
